REQUIREMENTS TO PARTICIPATE:
PHYSICAL - Complete one during middle school years. It must be on file in the school office.
ATHLETIC CONTRACT - Completed each school year.
INSURANCE - Athletes are required to sign a form acknowledging insurance coverage or purchase of school insurance.
ASB STICKER - Athletes are REQUIRED TO PURCHASE an ASB Sticker for their Student Body Card. ASB cost is $15.00. This helps cover the cost of officials and equipment.
